	Dr. Sidney B. Smith Dr. Smith is a Board Certified Dermatologist. He has vast experience in all aspects of cosmetic, medical, and surgical dermatology. Dr. Smith graduated from Brigham Young University, University of Utah, and with honors from Ponce School of Medicine. He completed his Transitional Internship at William Beaumont Army Medical Center, Fort Bliss and his Dermatology Residency at the San Antonio Uniformed Services Health Education Consortium at Brooke Army Medical Center, Fort Sam Houston & Wilford Hall Air Force Medical Center. During his over eight years of service in the United States Army, Dr. Smith has served as Chief of Dermatology, Assistant Chief of Dermatology, Teaching Staff for Dermatology Residents and other residents, and 12 months as the Operation Iraqi Freedom II Theater Dermatologist in Baghdad, Iraq. He has vast experience in Teledermatology. He has authored over 20 dermatology journal articles and chapters. Additionally he has given numerous academic dermatology lectures locally and nationally. 	Dr. Daniel P. McDonald Dr. Daniel P. McDonald grew up mostly in Florida and San Diego, California. He did his undergraduate work at San Diego State University. Graduating from medical school from the University of South Alabama, he also completed his residency there. He has been in private practice for almost eight years. He has grown his practice into one of the largest and most successful in a very short time period. He attributes his success to outstanding patient service and care. 	Dr. Thomas Moffo Dr. Moffo is a graduate of Indiana University at Bloomington with a Bachelor of Science. He graduated from Indiana University School of Medicine and completed his medical training in Internal Medicine at the Indiana University Medical Center. He has practiced emergency medicine in Arizona for the past 15 years. Dr. Moffo is the medical director for Dermacare at Arrowhead and has advanced training in BOTOXÂ® Cosmetic. His special interests are BOTOXÂ® and dermal fillers.



	Dr. Abraham J. Sayegh Dr. Abraham Sayegh was recently awarded the franchise rights to the Dermacare Laser & Skin Care Clinic located in North Scottsdale, Arizona. The clinic, currently scheduled to open in early December 2005, will be located in North Scottsdale along the prestigious Pima business corridor. Dr. Sayegh was originally introduced to Dermacare by a colleague who is involved with the Chandler, Arizona, Dermacare Clinic. Impressed with the professionalism, technology and safety record of the company, he decided that he also wanted to be part of the positive effects that Dermacare treatments can provide to young and old to improve the look of their skin. After opening a private practice in internal medicine in the Valley in 1989, Dr. Sayegh has continued to be very active with several medical committees and the community. Currently he is the proprietor and attending physician at North Valley Physicians Group which is affiliated with Paradise Valley Hospital. In addition, he serves as Medical Director for Hospice of Arizona, Chairman of the Department of Medicine at Paradise Valley Hospital and is the Medical Director for Ridgecrest Nursing Home. 	Dr. Timothy Bode Dr. Bode and his partner, Johnathon Munoz, M.D., own the Regional Support Center for a region in Alabama and Northern Florida. Dr. Bode attended the University of California-Davis for his undergraduate B.S. degree and received his medical degree from Tulane University Medical School in 1994. Dr. Bode currently serves as the Medical Director of Hospitalist Services of Central Alabama. Previously he served as an Assistant professor of Family Medicine with the Montgomery Family Residency program and in the U.S. Air Force as a physician with the rank of Major.



	Dr. Kenneth Egli Dr. Egli is a Board Certified Emergency Physician from Richland, WA. He is a graduate of Brigham Young University and Saba University School of Medicine. He completed his family practice internship at UT Southwestern, Parkland Hospital, Dallas, TX and his residency in Emergency Medicine at Herman Hospital in Houston, TX. Dr. Egli is a partner of Kennewick Emergency Physicians and is the Kennewick Fire Department Emergency Medical Services Director. 	Dr. Brian L. Fox Raised in Pennsylvania, Dr. Fox moved to Chicago for med school in 1993 and graduated from Chicago College of Osteopathic Medicine in 1997. He completed his Residency at Mount Sinai Hospital, Chicago Illinois. After graduating residency, he returned to serve as faculty at the residency. Dr. Fox is Board Certified in Family Practice (American Academy of Family Practice), focusing in on his practice and additional training in dermatology. He lectures to medical students and residents at Chicago medical schools and residencies on dermatologic diagnosis and treatment options. Dr. Fox is also actively involved in church and social justice organizations. He is currently practices in Family Medicine in Dyer Indiana. 	Dr. Gary K. Goforth Dr. Goforth, medical director for Dermacare Laser and Skincare Clinics of Tulsa, earned his medical degree from the Kansas City College of Osteopathic Medicine. He has practiced family medicine in Tulsa, Oklahoma for over 30 years. Dr. Goforth is Board certified in General Practice and has held leadership positions at Tulsa Regional Medical Center (formerly Oklahoma Osteopathic Hospital) as well as Oklahoma osteopathic organizations.



	Dr. Craig F. Ingber Dr. Craig F. Ingber is a Board Certified Head and Neck Facial Plastic Surgeon, with extensive laser experience during his 30 years at Kaiser Hospital in West Los Angeles, California. His practice included all aspects of head and neck surgery, including facial plastic surgery, microsurgery of the middle ear, laser surgery, cancer extirpation, and reconstruction of the head and neck. He is a former chief of the ENT/Head and Neck Surgery Department at the hospital. Dr. Ingber's medical degree was obtained at Temple University School of Medicine, where he also completed his internship and residency. He is a member of the National Medical Honor Society, and is a past President of the Babcock Surgical Society. He served in the United States Army for two years as Chief of Head and Neck Surgery at the 33rd Army Field Hospital in Wurzburg, Germany. Additionally, he is the Clinical Instructor at UCLA Medical School, Department of Head and Neck Surgery, and a Faculty Member, Division of Surgery at the Keck School of Medicine at the University of Southern California. Dr. Ingber has enjoyed performing surgery with a volunteer medical team in Guatemala. He is committed to community service and activism. He supports his wife, Mary, in her efforts as creator and chair of "Grateful Hearts" the popular Los Angeles Fire Department Fundraiser and Food Faire, and annual event that draws over 8,000 participants. They look forward to building their Dermacare Practice, and a legacy of community service and involvement, in the idyllic community of Valencia, California. After years of seeing patients with unpleasant plastic surgery results, Dr. Ingber is excited about the future of non-invasive, non-ablative skin care procedures such as those offered in Dermacare offices across the country.



	Dr. Suneil Jain Dr. Suneil Jain received his undergraduate degree in Biology from the University of Virginia and his Doctor of Naturopathic Medicine from the Southwest College of Naturopathic Medicine. He completed his clinical residency training in family medicine from the Scottsdale Natural Medicine & Healing Clinic.



	Dr. Michael Keverline Dr. Keverline is a graduate of the University of Notre Dame and he received his medical degree from the University of Pittsburgh Medical Center. He is a board certified Ophthalmologist who attended the Eye and Ear Institute of Pittsburgh. He has extensive experience with medical lasers, dermatologic injectables and oculo-plastic surgery. Dr. Keverline will oversee the medical direction of each Clinic.



	Dr. David M. Minozzi Dr. David Minozzi and his team are very proud to be awarded a Dermacare Laser & Skin Care clinic franchise in the Westchester, NY area. They are looking forward to introducing Dermacare to the greater Westchester area and are committed to helping Westchester men and women look their best. Dr. Minozzi, has a private chiropractic clinic in his hometown of Ardsley, NY. After graduating from Pace University he attended chiropractic school and has been in practice for the last 7 years providing quality chiropractic care for his patients. MINOZZI CHIROPRACTIC CENTER is one of the few chiropractic clinics able to incorporate chiropractic into a health spa setting. 	Dr. Dominic Moffo Dr. Moffo is a Phi Beta Kappa graduate of Indiana University at Bloomington. He attended Indiana University School of Medicine and completed his residency training at the Indiana University Medical Center. Dr. Moffo has practiced medicine for 15 years and is a diplomat of the American Board of Obstetrics and Gynecology. He is in charge of administrative operations for Dermacare at Arrowhead. 	Dr. Johnathon Munoz Dr. Munoz and his partner, Timothy Bode, M.D., own the Regional Support Center for a region in Alabama and northern Florida. Dr. Munoz attended Auburn University for his B.S. degree in Physical Science and received his medical degree from University of South Alabama College of Medicine in 1998 and graduated from the Montgomery Family Medicine Residency Program of Alabama in 2001.
