Biographies of our Doctors
Dr. Sidney B. Smith
Dr. Smith is a Board Certified Dermatologist. He has vast experience in all aspects of cosmetic, medical, and surgical dermatology. Dr. Smith graduated from Brigham Young University with honors from Ponce School of Medicine.
He completed his Transitional Internship at William Beaumont Army Medical Center, Fort Bliss and his Dermatology Residency at the San Antonio Uniformed Services Health Education Consortium at Brooke Army Medical Center, Fort Sam Houston & Wilford Hall Air Force Medical Center. During his eight years of service in the United States Army, Dr. Smith served as Chief of Dermatology, Assistant Chief of Dermatology, Teaching Staff for Dermatology Residents and other residents, and the Operation Iraqi Freedom II Theater Dermatologist in Baghdad, Iraq. He has vast experience in Teledermatology. He has authored over 20 dermatology journal articles and chapters.
Additionally he has given numerous academic dermatology lectures locally and nationally. Currently, Dr. Smith is on local and national pharmaceutical speakers bureaus. Dr. Smith is a member of the American Academy of Dermatology, the American Medical Association, and Alpha Omega Alpha.
Dr. Craig F. Ingber
Dr. Craig F. Ingber is a Board Certified Head and Neck Facial Plastic Surgeon, with extensive laser experience during his 30 years at Kaiser Hospital in West Los Angeles, California. His practice included all aspects of head and neck surgery, including facial plastic surgery, microsurgery of the middle ear, laser surgery, cancer extirpation, and reconstruction of the head and neck. He is a former chief of the ENT/Head and Neck Surgery Department at the hospital.
Dr. Ingber's medical degree was obtained at Temple University School of Medicine, where he also completed his internship and residency. He is a member of the National Medical Honor Society, and is a past President of the Babcock Surgical Society. He served in the United States Army for two years as Chief of Head and Neck Surgery at the 33rd Army Field Hospital in Wurzburg, Germany. Additionally, he is the Clinical Instructor at UCLA Medical School, Department of Head and Neck Surgery, and a Faculty Member, Division of Surgery at the Keck School of Medicine at the University of Southern California.
Dr. Ingber has enjoyed performing surgery with a volunteer medical team in Guatemala. He is committed to community service and activism. He supports his wife, Mary, in her efforts as creator and chair of "Grateful Hearts" the popular Los Angeles Fire Department Fundraiser and Food Faire, an annual event that draws over 8,000 participants. They look forward to building their Dermacare Practice, and a legacy of community service and involvement, in the idyllic community of Valencia, California.
Dr. Gael DeRouin

Dr. DeRouin is Board Certified in Facial Plastic Surgery and Otolaryngology. He is a well-accomplished physician with over 30 years of training and experience. Formerly, he has practiced in St. Louis, Mo. and Portland, Maine, where he completed his residency training at Maine Medical Center. He is a fellow of the American Osteopathic College of Otolaryngology-Head & Neck Surgery, as well as a member of the American Academy of Otolaryngology—Head & Neck Surgery and the American Academy of Facial Plastic and Reconstructive Surgery. He has made significant contributions to the Journal Osteopathic College of Ophthalmology & Otolaryngology, and he is currently working on a publication featuring Aesthetic Surgery of the Midface.
Dr. Sylvia Ramirez
Dermacare of Singapore
Dr. Ramirez is currently the vice president for global research and development at Arbor Research Collaborative for Health in Ann Arbor, Mich. She trained at Harvard Children's Hospital in Boston, is Board Certified from the American Board of Pediatrics and has designed and developed clinical programs, as well as practiced medicine in several settings — from academic clinical practice, to government clinics and the pharmaceutical industry — in both Singapore and the United States. Her experience provides her with unique insights in providing clinical care to various segments of the population. In addition, she holds an Executive M.B.A. from the University of Chicago Graduate School of Business and an M.P.H. in quantitative methods from the Harvard School of Public Health. A frequent contributor to peer-reviewed medical journals, Dr. Ramirez's background includes extensive concentration in the fields of pediatrics and nephrology.
Dr. Kevin B. Calhoun
Dr. Calhoun is a Board Certified Emergency Room Physician. Formerly, he managed urgent/emergency patient care, supervising 14 staff members at Sharp Chula Vista Hospital. Having received his M.D. from UCLA/Drew School of Medicine in 1993, he has also trained and supervised interns at one of the country’s busiest trauma centers (King/Drew Medical Center in Los Angeles), and also has a background in radiology and anesthesia. Dr. Calhoun is a member of the American Medical Association, the American Board of Emergency Medicine, the National Medical Society and the American College of Emergency Physicians.
Dr. Jeff C. Birchall

Dr. Birchall received his bachelor's degree and M.D. from the University of Calgary, Canada, and completed his residency at Foothill Hospital in the university’s Family Medicine program. He was a staff physician with the VA Hospital in San Diego and is currently physician (and past medical director) of the Penn Elm Medical Group in Escondido, Calif. He is Board Certified by the Canadian College of Family Physicians and the American Board of Family Practice.
Dr. Pamela DeRouin

Dr. DeRouin is Board Certified in Emergency Medicine, with a special interest in wellness and anti-aging medicine. She attended the University of Georgia, where she received a bachelor of science in biology, and the Medical University of South Carolina, where she received a Doctor of Medicine. She has been an emergency medicine physician for over 7 years, having completed her residency in 1999 at East Carolina University. She currently holds membership in the: American Medical Association, American College of Emergency Physicians, Emergency Medicine Resident Association and American Academy of Family Physicians, and she has contributed to the publication, Emergency Medicine, A Comprehensive Study Guide.
Dr. Abraham J. Sayegh

Dr. Sayegh is Board Certified in Internal Medicine. After opening a private practice in internal medicine in the Valley in 1989, Dr. Sayegh has continued to be very active with several medical committees and the community. Currently he is the proprietor and attending physician at North Valley Physicians Group, which is affiliated with Paradise Valley Hospital. In addition, he serves as Medical Director for Hospice of Arizona, Chairman of the Department of Medicine at Paradise Valley Hospital and is the Medical Director for Ridgecrest Nursing Home. He is also a Team Physician for the Harlem Globetrotters.
Dr. Phillip L. Barkley
Dr. Barkley completed his undergraduate degree at Southern Illinois University, Carbondale with a Bachelor in Psychology. He graduated from Southern Illinois University School of Medicine and completed a residency in Internal Medicine at Michigan State University. Dr. Barkley is Board Certified in Internal Medicine and is past-President of the American College Health Association. Dr. Barkley is also a member of the American College of Physician Executives.
Dr. Gary K. Goforth
Dr. Goforth is Board Certified in General Practice. He has spent over 40 years in Tulsa as a family practice physician and member of the Department of General Practice at Columbia Tulsa Regional Medical Center. Dr. Goforth is a graduate of Kansas State Teachers College and received his Doctor of Osteopathy Degree from Kansas City College of Osteopathic Medicine. His distinguished experience includes past President of the Tulsa District Osteopathic Society, Chairman of the Department of General Practice, Oklahoma Osteopathic Hospital, a Member of the Board of Trustees of the Oklahoma Osteopathic Hospital Founders Association, and Chairman of the Building Committee of Oklahoma Osteopathic Hospital during a multi-million dollar expansion program.
Dr. Kenneth Egli
Dr. Egli is a Board Certified Emergency Physician from Richland, WA. He is a graduate of Brigham Young University and Saba University School of Medicine. He completed his family practice internship at UT Southwestern, Parkland Hospital, Dallas, TX and his residency in Emergency Medicine at Herman Hospital in Houston, TX. Dr. Egli is a partner of Kennewick Emergency Physicians and is the Kennewick Fire Department Emergency Medical Services Director. Additionally, he is the owner and Medical Director of Mid Columbia Ambulance, Tri-Cities, WA. He is a member of the American College of Emergency Physicians.
Dr. Brian L. Fox
Raised in Pennsylvania, Dr. Fox moved to Chicago for med school in 1993 and graduated from Chicago College of Osteopathic Medicine in 1997. He completed his Residency at Mount Sinai Hospital, Chicago Illinois. After graduating residency, he returned to serve as faculty at the residency.
Dr. Fox is Board Certified in Family Practice (American Academy of Family Practice), focusing in on his practice and additional training in dermatology. He lectures to medical students and residents at Chicago medical schools and residencies on dermatologic diagnosis and treatment options. He currently practices Family Medicine in Dyer, Indiana.
Dr. Michael Keverline
Dr. Keverline is a graduate of the University of Notre Dame and he received his medical degree from the University of Pittsburgh Medical Center. He is a board certified Ophthalmologist who attended the Eye and Ear Institute of Pittsburgh. He has extensive experience with medical lasers, dermatologic injectables and oculo-plastic surgery. Dr. Keverline will oversee the medical direction of each Clinic.

Dr. Castillo-Plaza has practiced medicine in South Florida for nearly three decades, seen over 15,000 patients and delivered more than 5,000 babies. But for Dr. Castillo-Plaza, that's only the beginning when it comes to providing care for others. Dr. Suneil Jain received his undergraduate degree in biology from the University of Virginia and completed his doctorate of naturopathic medicine from the Southwest College of Naturopathic Medicine. He completed an additional two years general practice training at the Scottsdale Natural Medicine and Healing Clinic. Dr. Jain chose Dermacare Laser & Skin Clinics after careful research to determine how he could maximize patient safety while delivering proven results.
Dr. Jain is certified in the use of non-invasive lasers and has completed extensive clinical training in various non-surgical aesthetic techniques including dermal fillers, botulinum toxin type A, and advanced chemical peels. He is an innovator and CME-approved lecturer to physicians around the country. Dr. Jain is the medical director and co-owner of Dermacare Laser & Skin Clinic of Scottsdale/Fountain Hills.
Dr. Babak Nafe

Dr. Babak Nafe is a Naturopathic physician specializing in general and aesthetic medicine. Dr. Nafe received his undergraduate degree in Microbiology from Arizona State University and finished his Doctorate of Naturopathic Medicine from the Southwest College of Naturopathic Medicine.
Dr. Nafe's early interest in skin rejuvenation and cosmetic procedures has lead him to join the prestigious Dermacare Laser and Skin Care Clinics, where safety, efficacy and results are key factors used in every treatment. As an on site clinic physician, he has completed additional training in the use of dermal fillers, Botox cosmetic, medical grade chemical peels, intense pulsed light and advanced laser technology including laser hair, vein and tattoo removal. Dr. Guzman is an advisor to Dermacare of White Plains on effective skin treatments using minimally invasive procedures. Dr. Guzman graduated from Fordham University and got his medical degree from Ponce School of Medicine in Puerto Rico. He has been practicing in the Westchester and NY areas for 20 years.
